The Staying well Expert Patients Programme is a six-week course for anyone living with a long-term health condition. Its aims are to help you take control of your health by learning new skills to manage your condition better on a daily basis. Each session lasts for two and a half hours and is facilitated by trained tutors who are themselves living with a long-term condition. The Staying Well programme complements existing health care programmes and treatments, empowering participants to be more informed and better able to develop partnerships with their medical practitioners.
Topics covered by the programme:
Dealing with pain and extreme tiredness
Coping with feelings of depression
Relaxation techniques and exercise
Communicating with family, friends and professionals
Planning for the future
The Peer Support Group is open to anyone who has attended the Staying Well Programme. The aim of the group is for course participants to continue meeting and supporting each other. After completing the course it is very easy to feel lost and alone again, and to find it hard to remain motivated to continue with self-management techniques that you have developed.
Meetings at The Buttermarket, Retford Town Hall on the 2nd Thursday of the month 10.30am-2pm
The Connect Service- can help you by finding info about local services, activities & opportunities. If needed, Connect staff can work with you around money issues, housing problems, health management & other things. This short-term support will be tailored to your needs & will focus on achieving what you want in a way that suits you.
If after exploring these options you feel you may need more support, then you might need to have a Care & Support Assessment. This will allow you to explain the things you are finding hard to achieve & we will discuss with you how we might be able to help you.
The aim of the assessment will always be to help you regain or maintain a level of independence. We will carry out the assessment in the most proportionate way depending on your needs, this could be over the telephone or at one of our clinics.
Please contact us on 0300 500 80 80, an advisor can help you with your options and may complete a referral for a care and support assessment.
Newcomers always welcome. We arrange a wide range of weekly & monthly events for members & include MS drop in clinic, monthly newsletter, self help info, signposting for professional help, fundraising for local MS community, art class, neuro physio based Pilates exercise, swim/flot sessions, mels out & a monthly coffee cake & chat morning.
