The Connect Service- can help you by finding info about local services, activities & opportunities. If needed, Connect staff can work with you around money issues, housing problems, health management & other things. This short-term support will be tailored to your needs & will focus on achieving what you want in a way that suits you.
If after exploring these options you feel you may need more support, then you might need to have a Care & Support Assessment. This will allow you to explain the things you are finding hard to achieve & we will discuss with you how we might be able to help you.
The aim of the assessment will always be to help you regain or maintain a level of independence. We will carry out the assessment in the most proportionate way depending on your needs, this could be over the telephone or at one of our clinics.

Money Sorted in D2N2 provides support and personally tailored interventions for people experiencing the greatest financial difficulty. It will enable individuals to take control, build their confidence and skills and help them tackle barriers and problems in order to improve their financial well-being.
It is for people of working age who are experiencing financial difficulties and exclusion, who are unemployed or economically inactive and living in the D2N2 area.

Opportunity and Change is a support programme for people with multiple and complex needs throughout Derby, Derbyshire, Nottingham and Nottinghamshire. Our programme is being delivered by local organisations working together, with a range of experience of working with people with multiple and complex needs.
We aim to support people achieve stability and provide access to mainstream services. We work with individuals to create a tailored package of support that will assist with a range of needs including accommodation, mental healthcare and education.
We are part of the national Building Better Opportunities programme funded by the Big Lottery Fund and the European Union through its European Social Fund. It is being managed by Framework, a charity and housing association dedicated to helping homeless people, preventing homelessness, and promoting opportunities for vulnerable and excluded people.
